By submitting this form, I acknowledge and agree to the following terms:

A $100 non-refundable consultation fee is required. This fee may be applied toward future treatments if used within 3 months.

A $100 treatment deposit is required for each booked service. Reveal KBeauty Med Spa enforces a 48-hour cancellation policy. Cancellations made within 48 hours of the appointment, as well as no-shows, will result in forfeiture of the deposit. A new deposit will be required to schedule another appointment. The deposit will be refunded upon completion of the scheduled treatment.

Appointments are not considered confirmed until a confirmation text is received from Reveal KBeauty Med Spa.
